**Coastal Mom Movement — Mommy + Me Classes**



Every class is designed to meet you where you are — physically, emotionally, and in motherhood. Gentle, functional Pilates-inspired movement helps you rebuild strength, release tension, and bond with your baby.



**For moms with babies 6 weeks and up**


Strengthen your core and pelvic floor, move mindfully, and enjoy quality time with your little one. Babies are always welcome — in your arms, in a carrier, on a blanket, or just along for the ride.

**Bring:**
• Baby carrier (if wearing your baby)
• Water
• Baby essentials (blanket, milk, snacks, toys, seat, etc.)

LIABILITY WAIVER AND RELEASE OF LIABILITY

Coastal Mom Movement & the designated studio space
This Liability Waiver and Release (“Agreement”) is entered into by the undersigned participant (“Participant”), on behalf of herself and her minor child(ren) (“Child”), in connection with participation in classes and programs offered by Coastal Mom Movement and conducted at the designated studio space

By signing below, I acknowledge and agree to the following:

1. Voluntary Participation & Assumption of Risk
I understand that participation in Pilates, strength training, and Mommy and Me fitness classes involves physical activity that may be strenuous and carries inherent risks, including but not limited to:
• muscle strain
• falls
• equipment-related injury
• slipping or tripping
• injury related to babywearing
• injury occurring in studio premises, common areas, or parking areas
I voluntarily choose to participate and accept full responsibility for all risks, known and unknown, associated with participation for myself and my Child.

2. Acknowledgment of Postpartum & Prenatal Participation
I understand that postpartum and prenatal exercise carries unique considerations. I affirm that:
• I have received medical clearance to participate in physical activity, if necessary
• I am physically capable of participating
• I will modify exercises as needed
• I will stop immediately if I experience pain, dizziness, bleeding, or discomfort
I understand that instruction provided is not medical advice and does not replace care from my healthcare provider.

3. Child Supervision & Responsibility
I understand that Coastal Mom Movement provides fitness instruction only and does not provide childcare services.
I acknowledge that I am solely and fully responsible for supervising my Child at all times during class, including while exercising, babywearing, feeding, or moving within the studio.
I accept full responsibility for my Child’s safety, actions, and behavior during participation.

4. Babywearing & Equipment Use
If I choose to wear my Child during exercise:
• I assume all risks associated with babywearing while moving
• I affirm that my carrier is properly secured
• I understand that balance, stability, and load demands increase while wearing a baby
I understand that classes may involve use of light weights, resistance bands, Pilates props, mats, and other equipment. I assume all risks related to their use.

5. Use of Facility & Premises
I understand that participation includes use of the designated studio space, including but not limited to:
• studio floors
• equipment
• entryways
• restrooms
• hallways
• parking areas
• surrounding premises
I assume all risks associated with use of the facility and surrounding property.

6. Release of Liability
I hereby release, waive, discharge, and hold harmless:
• Coastal Mom Movement
• Cheyenne Heryford
• the designated studio space
• the facility owner and landlord
• property managers
• employees
• instructors
• contractors
• volunteers
• affiliated entities
from any and all claims, liabilities, demands, damages, actions, or causes of action arising out of or related to participation in the Program, including injuries sustained during or after participation.
This release applies to claims of negligence to the fullest extent permitted by law.

7. Indemnification
I agree to indemnify and hold harmless Coastal Mom Movement and the designated studio space from any claims, damages, or liabilities arising from:
• my actions
• my Child’s actions
• failure to follow instructions
• misuse of equipment
• unsupervised child activity

8. Emergency Medical Authorization
In the event of an emergency, I authorize representatives of Coastal Mom Movement or the designated studio space to obtain emergency medical treatment for myself or my Child if deemed necessary. I agree to be fully responsible for any medical expenses incurred.

9. Personal Property
I understand that Coastal Mom Movement and the designated studio space are not responsible for lost, stolen, or damaged personal belongings.

10. Photo & Media Release
I grant permission for Coastal Mom Movement to photograph or record me and my Child for promotional and marketing purposes, including website and social media use.
If I do not consent to media use, I will notify the instructor in writing prior to class.

11. Acknowledgment of Understanding
I confirm that:
• I have read this Agreement in full
• I understand its contents
• I understand that I am giving up certain legal rights
• I am signing voluntarily

This Agreement shall remain in effect for all current and future participation unless revoked in writing.
