WAIVER AND RELEASE
1. I understand that physical fitness training and conditioning of the type that I will engage in while working out with Crossfit  Central’s (CFC) trainers, either at CFC gym or with CFC trainers at other locations, is inherently dangerous. I also  acknowledge that Crossfit Central’s training methods are more rigorous and physically and mentally demanding than the type  of physical training that one receives at other gyms or facilities. Because of the inherent dangers that any type of physical  training entails and particularly because I am aware and acknowledge that CFC training methods are more physically and  mentally demanding, I understand and acknowledge that it is likely I may be injured while engaging in the type of training I may  receive while working with CFC trainers and using CFC equipment and facilities. I acknowledge and understand that the  injuries I may receive while working with CFC trainers include severe injuries that could result in permanent and disabling  injuries, paraplegia, quadriplegia or even death. Despite my understanding and acknowledgement of the inherent dangers  and risks involved in engaging in the type of physical and mental training with CFC trainers, I knowingly and willingly waive all  claims against Trinity Training, LLC, CFC, its trainers, whether those trainers are deemed to be Trinity  Training, LLC’s/ CFC’s “employees” or “independent contractors,” any other CFC employees or representatives and release  any claims I may have against Trinity Training, LLC, CFC, its trainers, whether those trainers are deemed to be  Trinity Training, LLC’s/ CFC’s “employees” or “independent contractors,” any other CFC employees or  representatives as a result of any injuries I receive while engaged in activities at CFC’s facility or at any other locations. I also  knowingly and specifically acknowledge that if I am asked or told to perform tasks by CFC trainers, whether those trainers are  deemed to be Boundless Energy Training, LLC’s/ CFC “employees” or “independent contractors,” that I believe to be  dangerous or that could result in injuries to me, I agree to not perform such activities; I acknowledge and agree that if I engage  in any physical training activities, I assume the risk of any injuries which may result. 
2. I knowingly and freely assume all such risks, both known and unknown, even if rising from the negligence of the releasees or others, and assume full responsibility for my participation. I release all teachers, students, and other parties from any claim  or any and all liability that may result from any injury received, and I hereby waive all claims that I, or anyone else on my  behalf, may make with respect to such injury or damages. 
3. I willingly agree to comply with the terms and conditions for participation. If I observe any unusual significant hazard during  my presence, or participation, I will remove myself from participation and bring such to the attention of the nearest official  immediately. 
4. I waive  and release any and all rights, claims and courses of action I have or may have against CFC or anyone associated with the  entity, it’s Primary Sponsor and it’s affiliates, their agents, employees, officers, directors, successors and assigns, the event  management company, the City, The Parks Districts, and any and all sponsors, their representatives and successors, that may  arise as a result of my participation in the event and any pre-and post-event activities. 
5. I attest and verify that I am physically fit and have sufficiently trained for the completion of this program, related event and/or  related activities, and a licensed medical doctor has verified my physical condition. 
6. I hereby grant full permission to any and all of the foregoing to use any photographs, motion pictures, recordings,  or any other record of this event for any legitimate purpose including commercial advertising.
